check spelling of 1st authority - I have transcribed as McMillan - even though the presence of a 'c' is dubious. On at least one other page in this volume someone has extended the top of the 'c' in red ink to make it clear it is 'Mc' (it was one of the 'Mcadam' entries). See also p.164 for both forms of 'McMillan'
